ELIGIBILITY:
Must be 18 years of age
Must have been a Lawful Permanent Resident for at least 5 years (or 3 years if married to a U.S. Citizen)
During the last five years have not been out of the U.S. for 30 months or more, and have not taken a trip out of the U.S. that lasted one year or more
Must have been a resident of the state from which you are filling your application for at least 3 months
DOCUMENTS TO BRING:
Permanent Resident Card (“green Card)
A check or Money Order for $680 (payable to Homeland Security)
2 Passport Photos
Birth Certificate with translation
Marriage Certificate (if married)
Proof of termination of marriage (if applicable)
If you have been arrested or detained by any law enforcement, for any reason: bring original or certified copy of Court Records
Names, dates and places of birth, including addresses of all children (with alien numbers if applicable)
List of employer names and addresses from the last 5 years, including dates of employment
Exact dates you have been outside the U.S. since you became a permanent resident
